It has achieved a lot in the areas of promoting and improving healthy living standards, footing the medical bills of those who need it and rehabilitating patients back into the larger society.

Job Title: Case Manager (Rehabilitation) Volunteer

Position Overview

  • As a Case Manager (Rehabilitation) Volunteer, you will play a vital role in supporting individuals through their rehabilitation process.
  • Working closely with clients, families, and interdisciplinary teams, you will provide holistic case management services aimed at promoting independence, recovery, and community integration.

Key Responsibilities

  • Conduct initial assessments to determine clients' needs, goals, and strengths.
  • Develop individualized rehabilitation plans in collaboration with clients, incorporating input from relevant stakeholders.
  • Coordinate access to necessary resources, including medical care, therapeutic services, housing, and community support programs.
  • Advocate for clients' rights and needs within various systems, such as healthcare, social services, and legal frameworks.
  • Monitor clients' progress, provide ongoing support, and adjust plans as needed to address changing circumstances.
  • Facilitate communication and collaboration among clients, family members, caregivers, and service providers.
  • Maintain accurate and up-to-date documentation of client interactions, progress notes, and service plans.
  • Participate in team meetings, case conferences, and training sessions to enhance skills and knowledge.

Qualifications

  • Bachelor's or Master's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related field (or equivalent experience).
  • Previous experience in case management, social services, or rehabilitation settings is preferred but not required.
  • Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds.
  • Excellent communication, advocacy, and problem-solving skills.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Compassion, empathy, and a commitment to promoting clients' well-being and autonomy.
  • Willingness to adhere to ethical standards and maintain confidentiality.
